Log my Care is a digital care management system covering care plans, daily notes, incidents, risk assessments and handovers, with bolt-on rostering, electronic medication records and a family communication app. It was founded in London in 2017 by Sam Hussain and Adam Hurst.

Its distinguishing move has been a free plan — unusual in a category where most systems are paid — which has made it a common first step for smaller providers moving off paper. It has raised roughly £6.25m across seed (2022) and follow-on (2024) rounds led by Mercia.
